Thebe fixes and features

Bug fixes:

  • Scratch cell not displaying widgets
  • thebe-init crashing
  • hide-input also hiding output
  • ipykernel no version info found
  • Pre-defined outputs break the Thebe cell generation process (fix is to remove them for now)

New features:

  • thebe-remove-input tag to replace remove-input, will also execute cell on initialization
