Syntax change to improve readability; output scalers are now labelled O: instead of looking like control scalers. Make mixer terminology more consistent; mixer inputs are 'controls'.

Now the mixer is a C++ library that can be fairly easily bolted into an output driver to provide mixing services. Teach the FMU driver how to use it as an example. More testing is still required.

Drop the X and + rotor mixers. We can't do multirotor mixing statically like that, as there is no mechanism for dealing with channel saturation.
